The WYSdom eSign tablet displays a patient questionnaire allowing patients to input their own personal and medical information. As a result there is a very significant reduction in practice administration as well as a substantial improvement in the accuracy of information held within the computerised patient record.

On subsequent visits to the practice the patient will be offered the opportunity to update any changes in their circumstances that have occurred between visits.

Each time the patient completes a questionnaire or revises their information the system securely dates and time stamps the electronically signed document.

Where appropriate WYSdom eSign facilitates electronic signature of a fully NHS approved digital FP17PR.

This removes the necessity of physical storage of thousands of paper FP17PR forms.
